The Department of Health and Human Services, Division of Medical Services, Office of Long Term Care is the licensure agency for Residential Care Facilities, Assisted Living Facilities--Level I and Assisted Living Facilities--Level II. Operators needing to conduct a Criminal Record Check must use the telephone system.  The online system was disabled by the provider.
