Services, Office of Long Term Care is the licensure
agency for Residential Care Facilities, Assisted Living
Facilities--Level I and Assisted Living
Facilities--Level II. Most of the regulations that cover
these facilities are available on the DHS website at

Residential Care Facilities, Assisted Living Facilities,
Level I and Assisted Living Facilities Level II are

Assisted Living Facilities
must also conduct an Adult Maltreatment Central Registry
Check on potential employees. The form and

Operators needing to
conduct a Criminal Record Check must use the telephone
system. The online system was disabled by the
provider.
The Health Services Permit Agency is the agency in
charge of determining need and awarding beds for the
different licensure types. Rules and regulations as well

Residential
Care Facilities that participate in the Medicaid
Personal Care Program will be required to use these
forms beginning March 1. This is the "Word"
versions of the forms.
